Accessing Your Health Records

West Tennessee Eye is pleased to offer you convenient, secure access to     your electronic health records through an online portal called RevolutionPHR.

This online portal gives you instant access to your West Tennessee Eye appointments, billing information, prescriptions, status of contact lens and glasses orders. 

RevolutionPHR utilizes the highest standards of online security while allowing you to view your information through any standard internet browser.

Steps to Access Your Electronic Health Records

1. Visit  www.revolutionphr.com 
   1. Click “Forgot Password”

1. On the next screen input your Username to receive a password reset link

• Using the password reset link sent to your email address, create a secure password using at least six characters long and include at least one number and one capital letter.

• Username for adults is the email address you have on file at WTE

1. 1. For patients under 21 years of age, the username is  

• Initial of first name + last name + birth year
• For example: John Smith born in at 2010 would be: jsmith2010
